Student Medical Profile on PAM

Over the past few of years, our current families will have been familiar with the use of CareMonkey as the database for their child’s medical information, and permissions for excursions/camps,etc.  From the end of 2018 we will no longer require parents to access CareMonkey as there is a new component in the Parent Access Module (PAM) in which to hold student’s medical information. This is a parent controlled electronic medical form and has many of the same features as CareMonkey.

From 2019, parents will use this new module of PAM to authorise their children’s participation in excursions.

All parents have been asked to fill in the medical information for their child. This is required before the end of the school year for current students, and prior to the Orientation Day on 3 December 2018 for the 2019 Year 7 students. This is a duty of care to enable all staff to be aware of your child’s medical needs while in their care.

If you haven’t completed the Medical Profile, you will need to log onto PAM and enter the health details of your child. Information on how to do this has been given to all families. If you have forgotten your login details to PAM, please contact our Database Administrator, Caroline Burge or email  pam@assumption.vic.edu.au

As outlined by our school Principal, the medical information you provide will be accessible by the school nurses and all relevant teacher(s)/supervisors for your child.
